This grant will help to support Patients in Transition: Providing Critical Interventions to Prevent Readmissions in High-risk Populations. This program is aimed to improve hospital-to-home hand-offs by providing 30 hours of intervention within 30 days of hospital discharge with the goal of reducing readmissions. It will provide a support system for individuals who have recently suffered from a stroke, traumatic brain injury or spinal cord injury, among other vulnerable populations at high-risk for readmission. This two-phase project will include a pilot program during the first year with the ultimate goal of fostering community partnerships with local hospitals and health organizations to offer patients a comprehensive team approach to transitions in the future. Grant funds will help to support programmatic equipment. This grant is funded through a cooperative agreement with the Centers for Disease Control and Prevention (Award # 1U59DD000338.)
