Application Process and Requirements
2012 Online Application Deadlines: March 9, 2012 and September 4, 2012
Notifications made late May 2012 and late December 2012, respectively.
STEP 1: Be sure you have read the Quality of Life guidelines (PDF). Applications that do not adhere to the guidelines will be rejected without review. Other helpful information is provided below.
To gain a clear understanding of the language that should be used when writing for programs for people with disabilities, please review:
STEP 2: Download the Quality of Life Grant Application form (doc) to your computer. When working on the application, please re-name the file and save as "Organization Name_City_State" (i.e., Reeve_Branchville_NJ). Detailed project budget instructions are included in the application guidelines. Prior grantees must include the final report on their most recent Quality of Life grant at the end of the Grant Application form (final report forms can be downloaded here).
STEP 3: Using the information contained in your completed Quality of Life Grant Application, click here to access the online application and complete the requested information (some repetitive information needs to be collected for our database). The online application may be completed in multiple sessions. Prior to completing the online application, you will be required to upload your completed Quality of Life Grant Application and a copy of your IRS 501(c)(3) Letter of Determination (if applicant is U.S.-based nonprofit). Attachments may be submitted in Word or PDF format. Access your saved online application here.
