筋萎縮性側索硬化症（ALS）に関する研究：筋萎縮性側索硬化症（ルー・ゲーリック病）は、一連の細胞関連の異変が起こることによって運動ニューロンの壊死（アポトーシスと呼ばれる）になる病気です。グルタミン酸（神経が信号を伝達するために必要な化学物質）の過多が果たす役割、ミトコンドリア（エネルギーを供給する細胞の一部）の異常、神経細胞伝達異常（蛋白質で伝達される信号や成長因子）、および、ウィルス説など、いくつかの手がかりを中心に研究が進められています。

破壊された神経細胞を取り替えるというアイデアは魅力的でかつ非常に希望の持てる考えですが、そもそも、新しい細胞がALSを引き起こした原因に対する抵抗力を持っているか否かについては不明です。しかし、幹細胞が運動ニューロンの識別子を取り込んで、筋肉の機能回復連絡網を形成するように思われています。幹細胞が神経栄養因子（ニューロンの成長や生存を補佐する脳内や筋肉内の蛋白質）の担体として働くので、これは確かに起こりうることです。上記の4つの因子に関する大規模な研究によってALS症状の改善が見られないことが証明されるまで、この分野の研究は有望視されていました。現在でも、これらの因子に関する研究は進められており、新しい方法が試みられています。

たとえば、2003年に、インスリン様成長因子1（IGF-1、マイオトロフィン）がALSによる衰弱の亢進を遅らせるか否かを調べるために、臨床試験が開始されました。IGF-1は、神経系統の正常な成長に不可欠であり、動物モデルでは運動ニューロンを保護する働きを持っていると示唆されています。これは細胞死の経路を阻止して、筋肉の刺激や軸索の成長や再生を促進すると考えられています。

臍帯血液／幹細胞：動物実験では静脈内の臍帯血液／幹細胞の臨床的役割が示唆されており、実際、ALS患者にこの細胞を投与した医療施設もあります。安全性に関する長期的試験結果や確固たる効果が出たという結果はまだ報告されていません。ALS協会には、投与後に体力や身体機能の改善が見られたという報告も寄せられています。さらなる研究の余地が残されています。

錠剤カクテル：ALSに関する最近のマウス実験では、リルゾール（米国食品医薬品局で認可されている唯一のALS治療薬）、ニモジピン（急性脳卒中や偏頭痛の治療に使用されるカルシウムチャネルブロッカー）、ミノサイクリン（炎症を阻止する可能性のある抗生物質）の複合投与が劇的な効果を示しています。複合投与によって細胞死を遅らせることができ、神経細胞の喪失が防止され、炎症が低減すると思われます。細胞死を阻止するプロテアーゼインヒビターや抗ウィルス剤を含む、神経の生存を補助する他の薬剤の研究が行われています。HIV患者やALS患者に、ウィルスを攻撃する「エイズ・カクテル」を投与した後、患者の症状に改善が見られています。これにより、一部の形式のALSはウィルス機構が関係していると示唆されます。

バスパー（欝病処方薬）やセレブレックス（関節炎処方薬）など、ALS以外のの理由で米国食品医薬品局に認可されている薬剤が、ALSでも試験されています。これらを投与することにより、ALS様症状のあるマウスの余命が延びることが判明しています。乳癌治療薬のタモキシフェン（ノルバデックス）も、マウス実験で良好な結果が得られています。現在、その臨床実験が実施されています。

情動変動（不意な笑いや泣き）の治療に対するAVP-923（dextromethorphan/quinidine）の効果を調べるため、臨床実験が行われています。また、ALS患者に対するクレアチン（筋肉のエネルギー代謝に関与して、天然に生成される化学物質）の効果に関する試験も続けられています。
