Loan Request for Library Materials

Please fill out the form to borrow books, videos, and audio books from the PRC. 

List the items you wish to borrow. Enter Title and Location Information for each item.
Location Information is located in the right column of the listing (a sample entry would be: RD 757.WA K37 1998).

There is a limit of 2 videos at a time, and 3 books at a time. Items are loaned for a 30-day period and must be returned. The PRC reserves the right to not loan out materials. 

Materials will be mailed to you with prepaid postage and a return address label. There is no cost to you for the return of the items. You may also save and reuse the box it comes in.

NOTE TO LIBRARIANS: The PRC does ILL through OCLC. Account name is CDRPR. We also do ILL in NJ through JerseyCat and our Velocity delivery number is 1513(Infolink).
