脊髓損傷 / Spinal Cord Injury

脊髓損傷

脊髓損傷(SCI)包括脊椎管內的神經損傷。大多數的脊髓損傷是由脊柱損傷引起的，脊柱損傷後，影響脊髓傳送和接收從大腦向人體控制感覺，運動，自主功能的系統發出的指令。

脊髓和大腦構成了中樞神經系統(CNS)，脊髓協調機體的運動和感覺。

脊髓包括神經元和稱為“軸突” 的長神經纖維。軸突沿下行通路傳導大腦信號或者沿上行通路把信號傳送到大腦。許多通路上的軸突大都包被著一種絕緣鞘，叫做髓鞘，呈白色，因此其所在的區域也稱“白質”。

神經細胞有一種樹狀的分支，叫做樹突，接收來自其他神經細胞的信號，構成“灰質”，位於脊髓中心的蝶狀區域。

和大腦一樣，脊髓也被三層膜（腦脊膜）包圍: 最內層是軟脊膜，中層是脆弱的蛛網膜，和強韌的外層：硬脊膜。

脊髓分為幾個節段，神經從各段連接人體的相應部分。頸段，從C1到C8，控制到頸，手臂和手的信號。胸段或上背部（T1到T12）傳輸信號到軀幹和手臂的一部分。肋骨以下的腰段或中背段（L1到L5）控制髖部和腿部的信號。最後，骶段（S1到S5）, 在中背部腰段以下，控制到腹股溝，腳趾和腿的局部的信號。不同節段的脊髓損傷反映這種組織結構。

有幾種細胞執行脊髓功能：運動神經元，有長長的軸突，控制頸，軀幹，四肢的骨骼肌；感覺神經元又叫背根神經節，其軸突構成了將機體資訊傳送到脊髓的神經，位於脊髓外部。脊髓中間神經元，完全位於脊髓中，幫助整合感覺資訊，生成協調信號，控制肌肉。

神經膠質，或支援細胞，在數量上大大超過了腦部和脊髓的神經元，具有許多必不可少的功能。少突神經膠質細胞，生成髓鞘，提高了神經信號傳遞的速度和可靠性。其他一些神經膠質像輪子的緣和輻一樣，圍住脊髓，提供了上行和下行神經纖維束的室。

星形膠質細胞調節神經細胞周圍的液體成份。一些細胞在創傷後會形成瘢痕組織。一些小細胞，叫做小膠質細胞，可能在創傷應激的時候被啟動，清除廢物。所有這些膠質細胞都產生物質來支援神經元存活，影響軸突生長。但是這些細胞也會阻礙損傷的癒合。

創傷後，周圍神經系統(PNS)將信號傳導到四肢，軀幹和身體的其他部分，其神經細胞或神經元可以自身修復，但是中樞神經系統不可再生。

大腦和脊髓的神經細胞對創傷的反應與身體其他細胞不同（包括周圍神經系統的細胞）。大腦和脊髓在骨腔中，骨腔有保護作用，但也使其非常脆弱，難於抵抗腫脹和外力引起的壓迫損傷。中樞神經系統的細胞代謝很快，依靠血糖提供能量，這些細胞要求充足的血供，才能正常工作。血流減少（缺血）時，中樞神經系統的細胞最易受損。

“血腦屏障”和“血脊髓屏障”是中樞神經系統的特性。這種屏障可以限制潛在有害物質和免疫系統細胞的進入，保護神經細胞。創傷可能突破這些屏障，導致大腦和脊髓的進一步損傷。“血脊髓屏障” 也會阻止一些藥物的進入。

最後，在大腦和脊髓，神經膠質和細胞外基質（細胞周圍物質）與外周圍神經不同。這種不同導致了其創傷反應的差異。

完全損傷與不完全損傷

“完全損傷”和“非完全損傷” 的區別是什麼？不完全損傷的病人在損傷水平以下還有一些感覺和運動功能，其脊髓還沒完全損壞和癱瘓。完全損傷的病人，其神經損傷阻止了大腦向身體各部位發出的信號。

脊髓損傷後也有完全恢復的可能，實際上，不完全損傷的病人恢復的機率更大。

據Craig醫院報道，在美國科羅拉多州一項關於脊髓損傷的大型研究中，七分之一傷後完全癱瘓的病人運動功能顯著恢復，但是那些傷後腿仍可運動的患者，約有四分之三的人恢復良好.

頸部受傷且能感受到腿上小別針刺痛的病人有三分之二最終恢復了肌肉力量，恢復行走。那些頸部受傷，只能感受到輕微觸覺的人，約有八分之一最後能夠走路。

肌肉越早開始工作，其恢復的機率就越大。而肌肉開始工作，在最初幾周，往往是在手臂而非腿部。

只要肌肉功能一有改善，可以預期更大的恢復。如長時間沒有起色，則恢復的希望渺茫。

統計學調查

在美國，大約有45萬人患有創傷性脊髓損傷，每年有10萬多新增病例。在所有的脊髓損傷患者中，男性82%，女性18%。

究其損傷原因，最常見的是交通事故，其次是跌傷和暴力損傷。運動傷多見於兒童和青少年，成年人脊髓損傷主要與職業有關（尤其是建築性工作）。

絕大部分的脊髓損傷患者都在十幾歲到二十幾歲，約80%是男性。65歲以後，男性的壓倒性比例降低，因為在這個年齡段，跌傷是創傷的主要原因。一半以上的脊髓損傷在頸部，三分之一在胸段（此處肋骨與脊柱連接），其餘在腰段，也就是下背部。

目前，對於脊髓損傷還沒有有效的治療方法。但是，關於手術治療和藥物療法的研究進展迅速。藥物控制損傷惡化，減壓手術，神經細胞移植，神經再生，合成藥物療法等正在被研究，試驗，以期治癒脊髓損傷。
